Program/Service Notes:
Case management services, Medicaid wavier, need total care. Program targeted to keep people in their own home. This a statewide program serves eight counties with a total of 69 slots, there is a waiting list. More information and phone numbers of other counties are available upon request. They are now serving clients statewide. Case management includes social and nursing assessments, care planning, referral, advocacy and monitoring.
