This program is designed to help elderly and disabled adults who need long term care, to remain at home and receive needed services as an alternative to nursing home placement. Eligibility: Medically or physically disabled adult, age 18 and over Elderly individual, age 60 and over Medically in need of nursing home care Financially eligible for Medicaid under special program guidelines Home care can be arranged at less cost than nursing home care
