Program/Service Notes:
Home Health program brings skilled care to homebound patients of all ages in Calhoun , Pleasants , Ritchie , Roane , Wirt and Wood County. We provide in-home care and counseling for patients with a terminal diagnosis in Pleasants, Ritchie, Wirt, and Wood County. Hospice staff handle all billing to Medicare/Medicaid reimbursement, private insurance,memorials, donations, and a generous and caring community. An interdisciplinary team of professionals and volunteers will develop with each patient/family/caregiver a plan of care which may include, as appropriate, the following services: Physician, Registered Nurse, Medical social Worker, Spiritual support Services, Physical, Occupational, and Speech Therapist, Home Health Aides, Volunteers, Nutritional support, durable Medical Equipment, Medications, Medical Supplies, Inpatient care, Counseling, and Family Bereavement follow up for twelve months after death. Housecalls Home Health provide skilled in-home nursing care for homebound patients under the direction of the patient's physician. SERVICES: Complete supervised nursing care, IV therapy, Pain Management, Personal Care, Total Parenteral nutrition, Chemotherapy, help with daily routine, Health education, Medical social work, Physical, Occupational and Speech Therapy. Counties with Home Health services include: Wood, Ritchie, Pleasants, Wirt, Roane, Calhoun, and Gilmer in W. VA. and Athens and Washington Counties in Ohio.
