Program/Service Notes:
In-home services provide help with personal hygiene, grooming, & other personal care; assistance with shopping & other errands; meal planning and preparation; assistance with walking; escort to medical appointments; transportation; range of motion exercises; light housekeeping and laundry; and companionship. Individuals can qualify for in-home services under two programs; (1) community Care - 18 and over; must meet certain requirements. (2) Medicaid Waiver - 18 and over; must meet certain requirements. Programs available are as follows: (1) III-B Services for those Senior Citizens 60+ Transportation, Chore, escort, repair/maintenance/renovation; shopping; assessments. (2) III-F Services - Health Services for 60+ (3)III-C-Meals Program (congregate and Home Delivered - In addition to a hot meal, there is socializing with others at the center through crafts, exercise program, domino, bingo, etc.; eligibility criteria for home delivered meals. Other Services provided are: Medical Equipment Full range of referral services telephone reassurance Insurance Counseling Non-Medicaid program Transportation (Open to the general public)
